It can sometimes be difficult for people to know what services are available to them. Pinecrest-Queensway offers a service to help clients navigate the health and social service system. Navigators are linked into each of the following departments, Children and Family Services, Community Health Services and Health Services.
The role of the system navigator is to engage clients with complex needs with the goal to increase client self-efficacy and autonomy. They do this by:
- assessing and determining client needs
- developing individualized service/care plans
- case conferencing and engaging supports
- making referrals to supports
System Navigators are available Monday through Friday by appointment. The service is available in both official languages and interpretation for other languages may also be arranged. For more information call general reception at 613-820-4922.
