West End Integrated Falls Prevention Program

The goal of the West End Integrated Falls Prevention Program (WEIFPP) is to reduce the number and impact of falls suffered by seniors living in the west end of Ottawa.


The program provides: 


  • Assessment and intervention for high-risk seniors 

  • Advocacy and identification of gaps in services for seniors 


How We Help


A team member is available to meet with each senior in their home to do a fall risk assessment and to identify risk factors. Together with the senior (and family and/or caregiver), a plan is created to reduce the risk of future falls. This plan includes home modifications, an in-home exercise program to improve balance and strength, and education about safety and healthy living. 


Who We Serve


To access the West End Integrated Falls Prevention Program (WEIFPP), seniors must live within the PQCHC area or the Western Ottawa Community Resource Centre catchment area. To find out if you live in the required catchment area, please enter your address at  http://www.coalitionottawa.ca/en/find-your-chrc.aspx.


Referrals are accepted from seniors who meet all of the following: 


  • Have had 1 or more indoor falls or many outdoor falls in the last year 

  • Are 75 years of age and older

  • Live in their own home or in a retirement home 

  • Are not currently receiving therapy services through The Home and Community Care Support Services or a day hospital

  • Are willing and able to take part in reducing their falls 


Services are offered Monday to Friday, 8:30 a.m. - 4:30 p.m. in clients’ homes. Services are provided in English or French with interpreters available for other languages.
