West End Integrated Falls Prevention Program

The West End Integrated Falls Prevention Program (WEIFPP) supports adults aged 70 and older is to reduce the number and impact of falls suffered by seniors living in the west end of Ottawa.


How we help:

A member of our interdisciplinary team meets with each senior in their home to assess their risk of falling and identify factors that may affect their safety and independence. Together with the senior (and family and/or caregivers when appropriate), an individualized plan is created to address their needs and goals, a plan is created to reduce the risk of future falls


What we provide:

  • Falls risk assessment and intervention for seniors at increased risk of falling

  • Personalized care plans developed with each senior, their family, and/or caregiver

  • Home safety recommendations and modifications to reduce fall hazards

  • In-home exercises to improve balance, strength, and mobility

  • Education on falls prevention, safety, and healthy living

  • Referrals and connections to appropriate health and community services

  • Advocacy and identification of service gaps to help improve access to supports for seniors


Who we serve:

To access the West End Integrated Falls Prevention Program (WEIFPP), seniors live within the catchment area (Pinecrest-Queensway Community Health Centre, Western Ottawa Community Resource Centre catchment areas, South Nepean, Carlington and part of the Nepean Rideau Osgoode Community Resource Centre). Referrals are accepted from seniors who meet all of the following eligibility criteria:

  • At least 1 indoor or outdoor fall in the past year

  • 70 years of age or older

  • Live in their own home or in a retirement home, NOT living in long term care facility

  • Cognitively and physically able and motivated to take action to reduce falls

  • NOT currently receiving other Falls related support services

  • Client/caregiver is aware of and consents to referral
