Our medical clinic provides primary care.
Primary care is delivered by family doctors and nurse
practitioners. Registered nurses, dietitians, and other healthcare professionals work as a team with your family doctor or nurse practitioner to help you stay healthy and get the care you need throughout your life. It is often the first place people go when they need health care.

Our Model of Care
At a community health centre, we care for the whole person. We know that health is affected by more than visits to a medical clinic. Things like safe housing, healthy food, income, education, social support and access to services can all affect a person’s health.
Our team works together to provide care, support and connections to community resources that help people live healthier lives.

Community health centres welcome everyone in the community. We have a focus on supporting people who may have a harder time getting the care they need. This may include:
People living on low incomes.
People from racialized and Black communities.
Newcomers, refugees and asylum seekers.
People from 2SLGBTQI+ communities.
People with disabilities, with mental health and/or substance use concerns.
How Can We Help?

Help you find a family doctor or nurse practitioner, at one of our sites or a partner clinic.
Connect you with health and community supports.
Provide services in your language.
If you receive primary care through our centre or one of our partner clinics, you may be able to access:
Mental health counseling

Chiropody (foot care)

Medication reviews and support from a pharmacist

Physiotherapy

Nutrition counseling with dietitian




Gender affirming care for gender diverse clients aged 17 and older, including hormone initiation, surgical referral, and post-op care
Women’s health services, including birth control and HPV testing
Lung Health Program that supports clients who have asthma or chronic obtrusive pulmonary disease (COPD)
A referral from your doctor or nurse practitioner is required to access these services.
Other Services Available to You
Pinecrest-Queensway and South Nepean Community Health Centres offer other health and community services. You do not need to be a primary care client to use these services.
