Neighbourhood Health Model
Information For Primary Care Providers and Clinics
Our Neighbourhood Health Model helps connect more people to primary care by increasing allied health and administrative supports in our clinics. This work is done in partnership with our network of community primary care clinics and mental health partners.
Our model has four key components.
Supported Attachment

In collaboration with the Ottawa OHT and Ottawa West Four Rivers OHT, we support patient attachment to primary care through streamlined, coordinated intake processes including offering system navigation supports.
Digital Health
We leverage ministry supported digital health tools in such a way to enhance the navigation and coordination of the delivery of primary care services.
Enhanced Team-Based Care
We increase access to allied health resources internally at Pinecrest-Queensway Community Health Centre and across network sites to grow capacity and ensure comprehensive care delivery to all patients.
Specialized Support
We connect patients from partnering community primary care clinics to specialized services (e.g., gender-affirming care, mental health, system navigation) or assume their primary care needs based on the unique patient’s profile.
