We're here to help!
System Navigation provides free support. We will listen to your concerns,

help you create an action plan, and connect you to resources and programs that meet your needs.

It can sometimes be difficult for people to know what services are available to them. System Navigation can help clients navigate the health and social service system.

Help you understand and fill out social service forms
Social prescribing
Provide service in 230+ languages through phone interpretation

Eligibility

In order to be eligible for this service, you must meet ONE of the following requirements:

Live within the PQCHC catchment area (Click here to find your CHRC)
Receive at least one other program or service at PQCHC
Be a patient of our medical clinic or at one of our partner clinics. Speak to your primary care provider for a referral.

Driving Directions

Pinecrest-Queensway Community Health Centre is located on Richmond Road, across from Lincoln Fields. If you are taking Highway 417, exit at Pinecrest and go north to Richmond Road. Turn right onto Richmond Road. PQCHC is the three-storey red brick building on the left just past Croydon Avenue and the Lincoln Heights car dealership.

Bus Directions

PQCHC is located roughly 500 meters from the Lincoln Fields transit station which is serviced by many major bus routes. Use the OC Transpo Route Planner to plan your trip to PQCHC, or call OC Transpo at 613-741-4390 for up-to-date route and schedule information.
