Our Model of Care
Community Health Centres are non-profit organizations who are committed to helping people improve their health and well-being. In Ontario, there are 75 community health centres, that work together to share ideas, resources and best practices to improve care for everyone. Pinecrest-Queensway Community Health Centre has two locations in Ottawa (Pinecrest-Queensway and South Nepean).

Community health centres welcome everyone in the community. We have a focus on supporting people who may have a harder time getting the care they need. This may include:
People living on low incomes.
People from racialized and Black communities.
Newcomers, refugees and asylum seekers.
People from 2SLGBTQI+ communities.
People with disabilities, with mental health and/or substance use.

Pinecrest-Queensway and South Nepean Community Health Centre
Pinecrest-Queensway Community Health Centre and South Nepean Community Health Centre provide primary care services for people in our community. We also work with other primary care clinics across Ottawa, so that you can get care in a location near you.
How Can We Help?
When you connect with us, we can:
Help you find a family doctor or nurse practitioner, at one of our sites or a partner clinic.
Connect you with health and community supports.
Provide services in your language.
If you receive primary care through our centre or one of our partner clinics, you may be able to access:

Physiotherapy

Nutrition counseling with dietitian

Mental health counseling

Medication reviews and support from a pharmacist




Gender-affirming care for trans and non-binary people aged 17 and older, including hormone therapy starts and support with surgery referrals
Women’s health services, including birth control and HPV testing
Lung Health Program that supports clients who have asthma or chronic obtrusive pulmonary disease (COPD)
Chiropody (foot care)
A referral from your doctor or nurse practitioner is required to access these services.
Other Services Available to You
Pinecrest-Queensway and South Nepean Community Health Centres offer other health and community services. You do not need to be a primary care client to use these services.

Creating a Safe and Respectful Space
Everyone deserves to feel safe and respected at Pinecrest-Queensway Community Health Centre.

Threatening, disruptive, or harmful behaviour will not be tolerated. This includes verbal, written, or physical harassment, threats, or actions that make others feel unsafe. Racism, oppressive language, and discriminatory remarks or actions toward staff, volunteers, students, or other clients are never acceptable.

These behaviours go against our values of equity, inclusion, and community well-being and may lead to consequences, including loss of service
For Primary Care Providers and Clinics
Our Neighbourhood Health Model helps connect more people to primary care by increasing allied health and administrative supports in our clinics. This work is done in partnership with community primary care clinics and mental health partners.
Our model has four key components.
Supported Attachment

In collaboration with the Ottawa OHT and Ottawa West Four Rivers OHT, we support patient attachment to primary care through streamlined, coordinated intake processes including offering system navigation supports.
Digital Health
We leverage ministry supported digital health tools in such a way to enhance the navigation and coordination of the delivery of primary care services.
Enhanced Team-Based Care
We increase access to allied health resources internally at PQCHC and across network sites to grow capacity and ensure comprehensive care delivery to all patients.
Specialized Support
We connect patients from partnering community primary care clinics to specialized services (e.g., gender-affirming care, mental health, system navigation) or assume their primary care needs based on the unique patient’s profile.
