West End Integrated Falls Prevention Program
The West End Integrated Falls Prevention Program is a
community-based team of healthcare professionals that works with
seniors, health service and community agencies to reduce the number
of falls and their impact on seniors, their caregivers and the
health care system through:
-
Assessment and intervention for high-risk seniors on
an individual basis.
-
Education, information and health promotion for
seniors and caregivers on fall prevention.
-
Best practices education for practitioners and
service providers.
-
Advocacy and identification of gaps in services for
seniors.
A
member of our team will meet with the senior in their home to
complete a comprehensive fall risk assessment, in order to identify
factors contributing to falls.
In collaboration with the senior (and family/caregiver) we develop a
plan of action to reduce the risk of future falls. This plan may
include home modifications, in-home exercise programs to improve:
balance and strength, and education regarding safety and health
promotion.
The program also provides education in the community for seniors,
caregivers and those who work with seniors, in partnership with
other agencies to promote awareness and best practices to reduce
falls.
The program accepts referrals from any source for seniors who:
Have had 1 or more
indoor or multiple outdoor falls in the last year;
and
Are 75 years of age and
older or have been identified as being at risk of falling if
younger;
and
Live in their own home
or in a retirement home;
and
Are not currently
receiving therapy services through CCAC or a day hospital;
and
Are willing and able to
participate in taking action to reduce falls
Referred seniors should be resident in the west of Ottawa (west of
Maitland). Referrals may be made to the program by calling
Pinecrest-Queensway Community Health Centre on 613-820-4922.
