System Navigation
It can be difficult for people to know what support is available to them. This service can help people better understand the health and social service system.
Services are provided Monday to Friday. Appointments are available
in-person, virtually, by phone, or video chat. Please call the centre to make an appointment with a System Navigator.

Support you in achieving the goals set out in your care plan

System Navigation is available to residents living within our catchment area or receiving at least one other program or service at our agency. Please refer to the coalition map to find the Community Health Centre or Community Resource Centre in your area. If you’re a patient of our medical clinic, speak to your nurse or doctor for a referral to System Navigation.
If you’re supporting someone who could benefit from meeting with a System Navigator, please don’t hesitate to call us or download the referral form below.
We are occasionally available upon request to provide outreach and education to local groups and agencies.

Driving Directions
Pinecrest-Queensway Community Health Centre is located on Richmond Road, across from Lincoln Fields. If you are taking Highway 417, exit at Pinecrest and go north to Richmond Road. Turn right onto Richmond Road. PQCHC is the three-storey red brick building on the left just past Croydon Avenue and the Lincoln Heights car dealership.
Bus Directions
